Forming Your Practice QI Team Our 6th immunizations Learning Collaborative cohort begins March 2024 and lasts 7 months. To participate, we ask that you assign at least two primary contacts to lead this QI project.
Clinical Leader This person should be a clinician, have a comprehensive understanding of how the clinic operates, and have a strong interest in quality improvement. The clinical leader will identify areas of opportunity and help launch the project amongst peers. The clinical leader does not have to be a physician but should have a direct patient care role. Examples: Physician, Nurse Practitioner, Physician Assistant, Registered Nurse.
Project Leader This person will serve as the project manager for this project at your clinic. The project leader will ensure materials are shared with clinic staff, the selected QI intervention is implemented, and data is shared with the Learning Collaborative leadership team. This leader should be comfortable with peer-to-peer instruction and have time to dedicate to the project. Examples: Physician, Nurse Practitioner, Physician Assistant, Registered Nurse, Medical Assistant, Administrative Staff.
Missed Opportunities Failure to vaccinate patients who are eligible to receive vaccinations at office visits
Data To assess improvement, we will collect HEDIS data for your clinic. We will share this data with you at project kick-off and quarterly for a year. We will also ask you to collect data on missed opportunities every other week and data on intervention implementation every month. We will provide you with simple tools to collect this data. We will use this data to help assess the impact of your intervention and modify it, if needed.
Optional Future Engagement The Learning Collaborative leadership team will support clinics beyond the 9-month cohort with quarterly WA-IIS HEDIS reports, faculty support, and QI coaching for an additional year. Clinics that opt into this program will be able to independently extend their QI project, implement additional QI projects, and monitor their outcomes.
